Lost Radio Contact Leaves Pilots on Their Own 20/12/2004
Without warning, on Tuesday, 14 Sept. 2004 at about 5 pm Pacific daylight time, air traffic controllers lost voice contact with 400 airplanes they were tracking over the Southwestern United States. Planes started to head toward one another and the controllers had no way to redirect the planes’ course. The crisis occurred at the Los Angeles Air Route Traffic Control Centre in Palmdale, which controls 460,000 square kilometres of airspace over Southern California. The unexpected shut down of the main voice communication system and the subsequent failure of the backup system disrupted about 800 flights. The air traffic officials stated had this happened 10-15 years ago when there was no onboard Collision Avoidance System, you would have had several mid-air collisions. A programming error in the software timer in the VCS was later found to be the cause of the malfunction. IEEE Spectrum, Nov. 04
Safe Servicing of Hubble 13/01/2005
IEEE-USA Urges 'Safe Servicing' of Hubble Space Telescope for Humankind WASHINGTON (30 December 2004) -- NASA should "strive to develop procedures, technology and equipment that would allow the safe servicing of the Hubble Space Telescope, including the possible use of tele-operated robots," according to an IEEE-USA position adopted by the organization's Board of Directors at its last meeting of the year. In its position adopted in San Antonio on 17 November, IEEE-USA stated: "NASA should continue planning and preparing for the [space shuttle] SM-4 [servicing] mission, while expert panels and the National Academy of Science develop their reports and the [servicing] issue is thoroughly reviewed." IEEE-USA stressed that it "supports exploring all possible avenues to prolong the useful life of the telescope for the benefit of science and humanity." http://www.ieeeusa.org
ESSS School Award for Safety 20/02/2005
A general Safety awareness training was delivered by ESSS Chairman to 20 teams of Year 9 students at St. Augustine School on 19th Jan. This was followed by a brief test treated as a competition. The winning team of four bright students Syra, Caroline, Dan and Georgina received their prizes from ESSS Chairman during their school assembly on 28 Feb. The Chairman congratulated the school and the students for outstanding results and encouraged them to continue their focus and efforts on improving safety in all aspects of school life. A more extensive Safety Award aimed at schools is being launched later in the year.
ESSS Systems Safety Training in China 18/08/2005
The ESSS delivered a three day training in Principles of Risk Assessment and Systems Safety Best Practice in collaboration with the Beijing Jiaotong University in China. The event was held at the end of a ceremony for the formation of a Safety Assurance and Certification Centre (SARAC.Org.CN) during 19-20 April 2005
